Veterinary Health Disclosure
I certify that my dog is in good health and has no known medical conditions that would prevent safe participation in treadmill exercise, including but not limited to heart conditions, respiratory issues, orthopedic injuries, or neurological disorders. I understand that it is recommended my dog be examined by a licensed veterinarian prior to participation.
Assumption of Risk
I understand that participation in canine treadmill exercise involves inherent risks, including but not limited to injury, illness, stress, or death. I voluntarily assume full responsibility for any risks associated with my dog's participation in services provided by Zoomies on Wheels.
Release of Liability
I hereby release, waive, discharge, and covenant not to sue Zoomies on Wheels, its owners, employees, contractors, or affiliates from any and all claims, demands, actions, or causes of action arising out of or related to my dog's participation, including negligence, to the fullest extent permitted by Idaho law.
Care, Custody, and Control
I understand that while reasonable care and supervision will be provided, Zoomies on Wheels is not responsible for illness, injury, loss, or damage to my dog except in cases of gross negligence.
Emergency Authorization
In the event of an emergency, I authorize Zoomies on Wheels to seek veterinary care for my dog at my expense if I cannot be reached. I understand that every effort will be made to contact me prior to treatment.
Behavior & Safety
I certify that my dog does not have a history of aggression toward people or equipment and agree to disclose any behavioral concerns prior to service. Zoomies on Wheels reserves the right to refuse or terminate service if a dog is deemed unsafe or overly stressed.
Photo & Video Release
I grant permission for photos or videos of my dog to be used for marketing and promotional purposes. If I do not want photos or videos of my dog used publicly, I can email zoomiesonwheels@outlook.com and Zoomies on Wheels will honor that request going forward.
